Medical Assistance
In order to be eligible for Medicaid, the person applying, called the "applicant" must provide information on the households' circumstances. The household must contain at least one of the following:
- A child under the age of 19
- An individual over the age of 64
- An individual considered blink or disabled through the Social Security Administration
- A pregnant woman
- The caretaker of a minor child(ren) living in the same household
Some services that Wisconsin Medicaid covers:
- Doctor visits, including prenatal care.
- Hospital care.
- Well-child and HealthCheck visits, including immunizations (shots).
- Vision and Hearing services, including eye glasses and hearing aids.
- Prescription drugs.
- Occupational, physical and speech language therapy.
- Mental health and substance abuse therapy services.
- Dental Services.
- Transportation to Medicaid services.
- Home care services.
- Institutional care.
- Wisconsin Medicaid offers several programs to help recipients pay for their Medicare Part A and/or Part B premiums if they meet certain income and other qualifications.
People must show their Medicaid card, called a "Forward Card", to their health care provider each time they receive a service. It is the person's responsibility to make sure the provider (doctor or other health care provider) accepts Medicaid before receiving any treatment.

A person can apply for Medicaid by mail, by phone or in person. If you think you may be eligible, contact the ES Division at 608-742-9227 now. You will receive a phone or in-person appointment or a mail-in application form depending on your preference.
