Behavioral Health

The Behavioral Health & Long Term Support Division's purpose is to work together to build positive community relationships with individuals and families to achieve their chosen potential. With one division administrator, four program coordinators, seven social workers, and one human services aide, we accomplish a lot together for individuals in our community who struggle with mental health issues, alcohol and/or drug addiction, or childhood disabilities.

 

Birth to 3 Program

The Birth to 3 Program is Wisconsin’s early intervention program for infants and toddlers, age’s birth to 36 months, with developmental delays or disabilities and their families. A federal law, the Individuals with Disabilities Education Act (IDEA), provides a framework for a comprehensive program for coordinating developmental health and social services within the family’s community.

Eligibility is based on a diagnosed disability or developmental delay of 25 percent in one or more areas of development determined through evaluation of 5 areas; Cognitive, Physical/Motor, Communication, Social/Emotional and Adaptive.

When a child is found eligible, the service coordinator will work the family and providers to develop an Individualized Family Service Plan (IFSP) that will outline outcomes and services to help support the child and their family at home and in their community.

All services are provided in a child’s natural environment; home or childcare setting. The Primary Coach approach is a practice that promotes parents’ and/or caregivers’ confidence and competence in supporting a child’s learning and development. A family will have one Primary Provider who develops the relationship with the child/family. The other team members will support the primary coach and family as needed.

Columbia County Department of Health and Human Services runs the Birth to 3 program. The Department contracts with CESA 5/Portage Project to provide the therapy services. Team members include the Service Coordinator as well as a Physical Therapist, 2 Speech Therapists, an Occupational Therapist and a Teacher.

Referrals can come from anyone: parent, doctor, daycare provider etc. It is important to get permission from the family to make the referral.

Referrals can either go through Central Intake (ext. 713; 742-9713) or can come directly to Bonnie Erickson, Service Coordinator (ext. 231; 742-9231) No referral form is needed; just a phone call.

The Service Coordinator will contact the family within 2 days to schedule a screening visit. At the initial screening visit, developmental history is gathered and a screening is completed using the Ages and Stages Parent Questionnaire to determine if further evaluation is necessary.

If evaluation is recommended, the Program has 45 days to complete the evaluation, determine eligibility and complete the IFSP. This timeline begins the day the referral/phone call was made to the Program.

CLTS & CCOP

The Children’s Long Term Support (CLTS) and Children’s Community Options Program (CCOP) are two of Wisconsin’s programs that help support children with a long term disability and their families.

CLTS is a Waiver that provides Medicaid funding for children who have substantial limitations in their daily activities and need support to remain in their home or community. Eligible children include those with developmental disabilities, severe emotional disturbances and physical disabilities. Funding can be used to support a range of different services based on an assessment of the needs of the child and family which can include: respite, adaptive aids, supportive home care, home modifications, daily living skills training, specialized medical and therapeutic supplies.

CCOP provides similar services but is a more flexible source of State Funding that can also cover other services such as recreational activities and some assistance with childcare or other services that may not be covered under the CLTS Waiver.

To apply for CLTS and/or CCOP a family needs to call Compass Wisconsin Threshold (CWT) 608-263-7861 to request the application. When the application is complete, the family contacts CWT again to schedule a home visit with a consultant to complete the initial assessment.

CWT is the doorway to apply for these long term support programs as well as the Katie Beckett Medicaid Program. Katie Beckett provides MA to a child who has a disability and is not able to qualify for Medicaid through BadgerCare or SSI due to their family income. Katie Beckett is based on need/disability of the child; parents income is not considered, however a child cannot have more than $2000 in assets.

Community Support Program (CSP)

Contracted through the Journey Mental Health, CSP provides community based services to adults with severe and persistent mental illness (such as schizophrenia, major depression, or bipolar disorder).

CSP services include 24-hour crisis intervention, symptom management, supportive counseling, medication management, psychiatric and psychological services, individual, group and family psychotherapy, rehabilitation services, employment and vocational services, activities of daily living training, social and recreational skills training, support services (access to services to meet physical or dental needs, legal needs, necessary transportation, financial support, and living accommodations).

Comprehensive Comm. Serv. (CCS)

In Columbia County, Comprehensive Community Services (CCS) is a voluntary, community-based program funded by the State of Wisconsin and operated by the Columbia County Department of Health and Human Services.

The CCS program offers a wide array of psychosocial rehabilitation services. These are services and supportive activities that assist CCS consumers with mental health and/or substance use conditions to achieve their highest possible level of independent functioning, stability, and independence, and to facilitate recovery.

The CCS program can provide a number of services needed at no cost to the consumer. Services that fall under the Service Array and are part of the consumer’s approved Recovery Plan are fully covered. The CCS Service Array includes the following areas:

  • Screening and Assessment
  • Service Planning
  • Service Facilitation
  • Diagnostic Evaluation
  • Medication Monitoring
  • Physical Health Monitoring
  • Peer Support
  • Individual Skill Development and Enhancement
  • Employment-Related Skill Training
  • Individual and/or Family Psychoeducation
  • Wellness Management and Recovery/Recovery Support Services
  • Psychotherapy
  • Substance Abuse Treatment

Basic eligibility criteria includes:

  • Columbia County resident
  • Eligibility for Medical Assistance
  • Have a current mental health and/or substance use diagnosis
  • Functional limitation in one or more major life activities caused by mental health or substance use issues as measured by the state screening tool
  • Need for psychosocial rehabilitation services
  • Must be able to obtain a current physician authorization for services

Crisis Services

Columbia County contracts with Northwest Connections to provide emergency mental health and substance abuse services on a twenty-four hour basis for residents of Columbia County. The Crisis Call Center is available 24 hours a day, 7 days a week, by dialing 1-888-552-6642. Mobile crisis services are also available when necessary to assess risk and assist in the development of a safety plan.

If you are concerned about your immediate safety or the safety of someone you know, please call 9-1-1 for immediate assistance. Law Enforcement often works closely with Human Services in crisis situations involving mental health, drugs or alcohol.

Developmentally Disabled Services

Developmental Disabilities include mental retardation, Cerebral Palsy, epilepsy, visual and audio impairments, and other neurological conditions. The disability must have occurred prior to the 21st birthday and be expected to continue indefinitely.

A case manager/service coordinator, who assists the person and the family or guardian, in assessing the needs and arranging for ongoing services, coordinates services for the developmentally disabled person in Columbia County.

Outpatient Therapy & Psychiatry

The County provides funding for outpatient therapy and psychiatry for adults and youth who do not have access to other funding for mental health or substance abuse services.  County funded therapy is intended to be short term in nature and for the purpose of stabilization.  To qualify, individuals mus be a resident of Columbia County, have a mental health or substance abuse diagnosis, and be experiencing difficulty functioning in their daily lives.  Individuals are asked to pay a monthly copayment for services based on their income.  We use federal poverty guidelines to determine the amount of copayment.  We are currently contracting with the Pauquette Center and Aspen Counseling for outpatient services.

OWI Court

Columbia County OWI Treatment Court is a post-conviction criminal court that attempts to achieve a decrease in recidivism and substance abuse, the purposes of which will increase the chances of successful habilitation to its participants. OWI Treatment Courts are built upon a unique partnership between the criminal justice system and the treatment community, one which structures treatment intervention around the authority and personal involvement of the Judge. OWI Treatment Courts are also dedicated to the creation of an unbiased courtroom atmosphere where a single Judge and a dedicated treatment team work together toward a common goal of breaking the cycle of alcohol abuse and criminal behavior.

The Columbia County OWI Treatment Court mission is to reduce the number of repeat intoxicated drivers by providing participants with comprehensive long-term treatment and intensive case management. The program will use evidence based practices to encourage recovery, stability, and accountability in order to decrease cost to our community, promote public safety, and reduce acts of criminality.

This program will encourage recovery through a coordinated response to participants who are dependent on alcohol or codependent on other drugs. Evidence based interventions assist participants who have court involvement and behavioral health issues as well as other complex needs to develop the necessary skills to successfully address both public safety and individualized participant case planning. The skills learned will reduce confinement time and support successful community tenure and recovery, without compromising public safety.

Respite Services

Respite is available to aid the families and caregivers with the care of the disabled individual. It allows for time out from the day-to-day care and the supervision necessary to maintain developmentally disabled people in their homes.
