Prenatal Care Coordination

What is Prenatal Care Coordination?

Prenatal Care Coordination is a Medicaid and BadgerCare+ benefit that helps eligible pregnant women get the support and services they need to have a healthy baby.  Your nurse will work with you during your pregnancy and for up to 2 months after your baby is born.  There is no cost to you for this service.

What can I expect if I enroll?

Women enrolled in Columbia County’s Prenatal Care Coordination program can expect:

  • One-to-one visits with a nurse, usually once per month, but there can be more frequent visits if you would like.  Visits can occur in your home, at the Health and Human Services building, or another mutually agreed upon location.  Just ask your nurse!
  • Information about pregnancy and parenthood tailored to your specific interests and learning needs.
  • Screening for pregnancy-associated depression and anxiety.
  • Support with breastfeeding your baby.
  • Connections to community and personal resources that meet your individual needs.
  • Personal and family support.

Columbia County’s Prenatal Care Coordination program DOES NOT replace your prenatal medical check-ups with your doctor or nurse-midwife, but rather gives you more time and information to prepare you for your new baby. 

How do I know if I qualify for this program?

A nurse will meet with you and ask you some questions to see if you meet eligibility requirements for the Prenatal Care Coordination program.

Why does Columbia County Division of Health offer this program?

Healthy moms have healthier babies, and we want all babies in Columbia County to have the best start in life possible.  Every pregnancy is unique and can present its own challenges, so even if you have been pregnant before this program can still be helpful to you!

  • Work with you to provide a personalized, confidential plan of care.

 

How do I get enrolled in Prenatal Care Coordination?

  • If you live in Columbia County, you can call 608-742-9227 and ask for Intake.

  • You can call The Well Badger Resource Center at 1-800-642-7837.

  • You can call Medicaid Member Services at 1-800-362-3002 or TTY 1-800-947-3529.

      

Medical providers may refer pregnant women for Prenatal Care Coordination services using the Prenatal and Postpartum Referral Form.
