Columbia County provides targeted case management for adults and youth who have a funding source such as Medicaid or private pay.  To qualify for case management, an individual must have a mental health or substance abuse diagnosis which significantly impacts their daily life.  The social worker will complete an assessment and work with the individual to develop a plan for how to meet their needs.  The social worker will then assist the individual with gaining access to needed services and will monitor progress towards the individual's goals.
